The purpose of this course is to enable to learner to recognize the impact of 30-day readmissions on the healthcare system and interventions from the evidence that have been used in an effort to prevent them. Research has found common characteristics and commonly chronic diseases among people who are at risk for readmission. Implementing post-discharge interventions, including the integration of a pharmacist and active engagement at the time of discharge, are discussed.
