Effective management of older adult health requires all team members to align care based on the patient's needs, implement evidence-based practices, and do no harm. This complex process can be simplified by implementing the 4Ms framework. This framework is the basis of Age-Friendly Health Systems and over 2000 healthcare systems have adopted it across all settings and disciplines since 2017. Rehabilitation therapists who understand the goals of Age-Friendly Health Systems and how to integrate the 4Ms framework play an important role in achieving the best outcomes for their patients. This course describes the 4Ms framework, how to apply the framework to your practice setting, and the role and value you can bring to your team and patients by integrating the 4Ms into patient care.

Christopher Meketansky OTR/L graduated from Stony Brook University in 2017 with a Master of Science Degree in Occupational Therapy. He received training in manual therapy, specialized treatment for individuals with degenerative neurological disorders and post-stroke neurorehabilitation. He has worked in skilled nursing and home healthcare settings with experience treating patients with dementia, Alzheimer’s disease, Parkinson’s disease, balance impairments, and a myriad of other conditions impacting the older adult population.
