Over the past several years, there has been a shift in post-acute care from a medical care focus to a person-centered approach that focuses on individuals’ personal preferences and culture. The primary elements of person-centered care include knowing the person, maximizing the person’s choices, and providing a supportive environment for the person to thrive. By focusing on a person’s preferences and culture, their quality of life can be significantly improved. This course discusses culture change and the positive benefits of providing person-centered care.
