People with mental health and substance use disorders are at higher risk for undesirable health outcomes and health disparities compared to other parts of the population. There are many factors that can influence an individual’s health outcomes, such as comorbidities, the fragmented healthcare system, or social determinants of health. However, there are also strategies to address those factors, mitigate risks, and prevent undesirable health outcomes. People with mental health and substance use disorders may need extra support to address these issues.
Case management and care management are two approaches that can support integrated care and help clients with multiple needs navigate various systems, reduce healthcare costs, and ultimately improve client health outcomes.
The goal of this course is to provide case management/care management, addictions, behavioral health counseling, marriage and family therapy, psychology, social work, and nursing professionals working in health and human services settings with education about factors that impact health outcomes of the behavioral health population, case and care management, and strategies for comprehensively managing multiple client needs.

Jennifer received her Bachelor’s degrees in Social Work and Criminal Justice from the University of North Carolina at Charlotte and Master’s degree in Social Work with a Certificate in Substance Abuse Studies from the University of North Carolina at Chapel Hill.
