Medication Calculations, Opioid Conversions & Titration: A Practice Review

 

Opioid conversion consults are one of the most common received within the clinical call center in addition to requests for drug information on newer agents such as Nucynta, Exalgo and Buprenorphine dosage forms. Seventy-five percent of use of our internal drug information service relates to symptoms of pain and opioid conversions. In addtion, annual client survey results reveal pain management as a popular topic and an area where nursing seeks to improve skills.

Demonstrate the use of an equianalgesic dosing table
Identify the pharmacologic, pharmacokinetic, and cost differences of various opioids and dosage forms
Initiate opioid therapy or convert between different opioids, dosage formulations, or routes of administration in an actual or simulated patient

As the Education Manager and Call Quality Coordinator at Enclara Pharmacia, Natalee is responsible for managing Enclara Pharmacia’s internal education services. Her role includes overseeing new hire pharmacist training, pharmacy student internship activities, and the call quality program, as well as participating in the drug information service. Natalee earned her doctor of pharmacy degree at Wilkes University, Nesbitt School of Pharmacy. She completed a 1-year post-graduate pharmacy practice residency program at Hahnemann University Hospital and maintains certification as a Board Certified Pharmacotherapy Specialist (BCPS).
