Determining Appropriate Level of Placement

As a member of the interdisciplinary team, PT, OT, and SLP professionals play a critical role in helping to determine appropriate level of placement and care within the post-acute care continuum. Many factors that are addressed by therapy professionals weight heavily on decisions for placement and care. As such, therapy professionals must understand why determining appropriate placement/level of care is important, the factors to consider when making these decisions including patient/family/facility goals, regulatory/reimbursement considerations, and assisting in facilitating smooth transitions between levels of placement and care.
