Hospice Referrals 

Hospice care addresses distressing physical symptoms as well as psychological and spiritual needs of persons at the end of life. Hospice also addresses the needs of these individuals’ families. Due to the complexity of issues at the end of life, optimal utilization of the hospice benefit can only occur if there is adequate time for the hospice to work with individuals and families.  This course will provide guidelines for how to make a timely, competent hospice referral. The major topics covered in this course are identifying hospice-eligible individuals, Medicare guidelines for hospice eligibility, communicating about hospice, and initiating a hospice referral. This course is intended for nurses and social workers with entry level or no hospice experience.

Instructor: Susan Heinzerling, RN, CHPN
Susan Heinzerling began her nursing career as a Med/Surg nurse. Renal patients in acute care, with multiple chronic conditions and repeated hospitalizations inspired her practice of hospice nursing where she began as Hospice Admissions Nurse and later became a Hospice RN Case Manager. Susan has practiced hospice nursing in both a large, urban area and in a rural setting. Susan earned a B.S. in Biology and B.S. in Nursing from the University of North Carolina at Chapel Hill and a Master's degree in Physiology from North Carolina State University. She became a Ph.D. Candidate in Immunology at the University of South Alabama. Her research experiences in the biomedical sciences led to an interest in evidenced-based practices.
