Unanticipated negative outcomes sometimes occur in the course of medical care. They are always difficult to explain to patients and their families. When the problem involves an error by a clinician or clinical system, the discussion is even more difficult. An error is defined as "the failure of a planned action to be completed as intended or the use of an incorrect plan to achieve an aim." Errors can include problems in practice, products, procedures, and systems. Disclosing errors involves identifying whether an event qualifies for a disclosure discussion with the patient.
