Communication is one of the most important elements of excellent care. This is especially true when decisions about medical treatments need to be made. Advance care planning is an ongoing process in which people explore and discuss their goals, values, and understanding of their health to guide future end-of-life treatment decisions. Unfortunately, most people do not engage in these conversations with their care providers, and end-of-life preferences are often unknown. It is important that all healthcare staff in all care settings understand how to engage in these conversations so that however a person arrives at the end-of-life, whether through accident or illness, their wishes are known and documented, and their families are well-prepared to make end-of-life decisions. This course provides an overview of the benefits and components of effective advance care planning conversations. You will learn when and how to start conversations and how to use a framework called FIVE WISHES® that simplifies and personalizes the end-of-life conversation and helps to elicit valuable information for decision-making. NOTE: This course is Part 1 of a 3-part series on advance care planning designed to build off one another. Part 2 is called Advance Care Planning Communication Skills for Success: The Five Wishes Framework and Part 3 is called Advance Care Planning Program Implementation: The Five Wishes Framework.  This course is appropriate for all healthcare team members.

Instructor: Kathleen Taylor
Kathleen Taylor serves as the Healthcare Advisor for Aging with Dignity, the national nonprofit organization that created and disseminates Five Wishes resources. Kathleen has over 27 years of experience in the healthcare and nonprofit sectors. Her passion is centered in improving the healthcare experience for people who are sick and vulnerable, and helping healthcare professionals master skills in cultivating empathy and communicating compassionately. She operates a healthcare consulting practice focused on palliative care strategy and programming. Previously, she served for a decade as the Director of community engagement at the country’s largest nonprofit hospice organization where she led their advance care planning education programs and developed their first training program on empathy and compassion in patient care. She also worked for several years as a hospice counselor serving patients and families coping with life-limiting illness, caregiving, and grief. Kathleen is a Florida Licensed Mental Health Counselor and Qualified Clinical Supervisor. She earned a Bachelor’s degree in Psychology at Stetson University, a Master’s degree in Mental Health and Rehabilitation Counseling at the University of South Florida, and a Graduate Certificate in Nonprofit Management & Innovation at the University of Tampa’s John Sykes College of Business.
