The Centers for Disease Control and Prevention states that as of 2012 about half of all adults—117 million people—had one or more chronic health conditions. 1 in 4 adults have 2 or more chronic health conditions (Centers for Disease Control and Prevention, 2017). This course provides an in-depth and interactive analysis of comorbidities and how they affect the health and well-being of patients. You will learn about cost effectiveness, social determinants, and nutrition needs related to comorbid conditions. You will also review action plans with patients that have comorbidities.
