People living with hypertension, coronary artery disease (CAD), and congestive heart failure (CHF) are among the highest cost members to a healthcare payer. People may have 1, 2, or all 3 of these conditions simultaneously and having one of these often increases the risk of developing the others. This is especially true if these conditions are not well-managed. This course will provide an overview of how to provide clinically-appropriate care, teach patients self-management techniques for their conditions, how to work with patients to create an action plan, and identify other factors that could impact their condition.
