Care Management to Increase Access and Decrease Readmissions

 

In the United States, it is estimated that about 6 out of every 10 adults have at least one chronic condition (Centers for Disease Control [CDC], 2018). This course will look at a two-part problem faced with providing care for this population. Some of these chronic conditions can be prevented or controlled in a better way. However, some payment systems have not allowed providers to profit from managing chronic conditions.
Today primary care providers have the ability to better manage patients with chronic health conditions by utilizing care management. Studies have shown that care management is effective in managing chronic health conditions (Eggington et al., 2012 ). Care management is also effective at managing those who have recently been discharged from the hospital. This course was written to inform you about using the care management model in the ambulatory care setting to increase a patient’s access to providers and decrease hospital visits, including readmission.

Elizabeth Kellerman MSN, RN started in nursing in 2007 after graduating from Samuel Merritt University in Oakland, California. While working in a critical care unit at Sutter Mills-Peninsula Hospital she earned her CCRN and TNCC. Her experience and knowledge led her to nursing education where she taught at a community college while working to receive her Master's in Nurse Education at Western Carolina University. As a nursing instructor, she spent time in many types of care settings including medical-surgical inpatient and community living centers.
