In addition to clarifying the eligibility criteria based upon MAC-specific LCD guidelines, this 8-part series will describes best practices for assessing and effectively documenting clinical indicators to substantiate eligibility, recertification and medical necessity. This video-streamed online series is designed for all interdisciplinary team members and uses case studies to describe the disease-specific criteria required for hospice admission and recertification based upon Local Coverage Determination (LCD) guidelines for CGS/NHIC/NGS and Palmetto. Learning Objectives:
Define the CMS requirements for hospice eligibility.
Name the clinical data points necessary to substantiate hospice eligibility for patients with heart disease, dementia, renal disease, liver disease, COPD/pulmonary disease, HIV/AIDs, stoke and coma, and Adult Failure to Thrive based upon LCD Guidelines.
Identify key components of documenting hospice appropriateness for each of the disease states above.
