Identifying medication regimens and assessing medication management are critical to creating an effective plan of care (POC). This module is a part of a series that will, not only walk you through those changes but provide guidance on interpreting those assessment items, existing and new ones. This module will review information formerly gathered from the medication sections of the OASIS-D1 and how it corresponds to sections of OASIS-E, as well as guidelines for accurately completing these items. Note: Unless otherwise stated, the information in this course was sourced from the OASIS-E Guidance Manual published by the Centers for Medicare & Medicaid Services (CMS, 2022).

J’non Griffin is a 34 year veteran of home care as an RN. She received her Master’s Degree in Health Care Administration in 2005. She has experience as a field nurse, director and executive with home health and hospice agencies, both large and small.
