Population health is an organized effort to improve the health of populations in similar geographical areas, with similar diseases, or health conditions. It is described as a bridge between clinical and public health. Population health provides opportunities for several different types of private, public, and other government sectors to work together for the better health of the community. It is influenced by the community in an effort to address social determinants of health (SDOH) so care can be delivered at a better level (Centers for Disease Control and Prevention [CDC], 2020).
