A speech-language pathologist (SLP) working with patients in the home has the opportunity to integrate functional and practical interventions with patients, which can increase patient accountability and improve outcomes. To effectively leverage the home environment, you need to quickly assess the high-value activities for your patient and develop interventions that use functional communication. This podcast describes best practices in assessment, effective interventions, and how to best prioritize a functional intervention for a specific patient to achieve the best outcomes. Documentation examples will also be provided, along with discussing how this approach contributes to perception of the SLP’s value on the home health team.
