The Healthy People 2030 campaign categorizes social determinants of health (SDOH) into five domains and each one has an impact on health outcomes and informs best practices in the delivery of healthcare (U.S. Department of Health and Human Services, n.d.). This course provides practice scenarios for use of a rapid assessment tool for screening patients for social determinants that impact their health.

Writer: Rebecca Smallwood, MBA, RNStephanie was educated and trained in New York State as a Licensed Practical Nurse where she practiced pediatric hematology/oncology nursing at the Children's Hospital of Buffalo and has over 25 years of clinical nursing experience. She earned her Associate in Science in Nursing, Bachelor of Science in Nursing, and Master of Science in Nursing with a concentration in Nursing Leadership and Administration from Excelsior College. She is a Certified Pediatric Nurse (CPN), Pediatric Advanced Life Support (PALS) certified, and is a former PALS instructor. Her clinical expertise is in acute pediatric medical surgical nursing and case management for children with medical complexity. She most recently worked at Duke Children's Hospital as a Nurse Clinician before coming to Relias.

Rebecca Smallwood, MBA, RN, has experience across multiple healthcare settings including: rural and urban acute care hospitals, public health, ambulatory surgery, higher education, and commercial healthcare learning organizations. Prior to joining Relias as an acute care SME Writer in 2017 and Curriculum Designer in 2021, she provided consultation to Swank Healthcare and was the Director of Education Services for Health.edu, a division of Texas Tech University Health Sciences Center. Rebecca earned a diploma in nursing from Methodist Hospital School of Nursing in Lubbock, Texas in 1989 and an MBA from Wayland Baptist University in 2010 with an emphasis in healthcare administration.
