Root cause analysis (RCA) is an approach to investigating adverse events. Its purpose is not to call out individuals who are at fault, but to identify the factors that make such events possible to begin with. Root cause analysis is a team process of discovery. It uncovers information about what happened in order to gain a detailed understanding of why it happened, and to make recommendations that will prevent it from happening again. This course provides staff with an overview of root cause analysis and some of the most common tools used to conduct it.
