This is even more important for older adults who are frail, deconditioned, or recovering from illness or surgery. This module describes the changes in nutritional needs that occur with aging and current nutrition guidelines for these high-risk populations. Learners will gain an understanding of the nutritional requirements necessary to promote health and wellness across the continuum of care, how to screen for malnutrition and connect patients to appropriate resources, and how to educate patients and caregivers on the important role nutrition plays in healing.
