Person-centered care plans are a communication tool for all members of the healthcare team. This course describes person-centered dementia care and how it applies to creating a care plan. It also explains why preferences and routines are important in the development of the care plan. The goal of this course is to teach administrators and direct care workers in post-acute care about person-centered care plans.
