Avoidable readmissions are caused in large part by adverse medication events. Up to 50% of adverse medication events can be prevented which can, in turn, lead to fewer hospital readmissions (Lemos et al., 2021). While there are many causes of adverse medication events, some can be prevented through medication management. Factors that can increase readmission include inappropriate medication administration and medication non-compliance. As an ambulatory care provider, you can help mitigate these problems through medication management. The goal of this course is to educate healthcare professionals in ambulatory care settings on the medication events that can be prevented through medication management.
