Advance care planning is an important service that is appropriate in all adult healthcare settings. It addresses many of today’s healthcare challenges and creates numerous benefits for care recipients, their families, and healthcare providers. Often, advance care planning activities in health systems occur without consistency, and the outcomes of these efforts are not measured or celebrated.
A solid advance care planning program includes systems, processes, and standards to help ensure that all care recipients have the opportunity to express and document their end-of-life treatment decisions, so that the care they receive is aligned with their values, goals, and preferences.
This course will help you determine the steps you need to take to build and implement an effective advance care planning program, and how resources from the Five Wishes program can help. You will learn how to build support, determine goals and measures, and identify processes and standards to ensure your program is making a difference for your care recipients and your healthcare organization.
