This webinar defines and explores prognostication in the context of hospice and palliative care. The importance of prognostication in allowing for life planning and providing a framework is assessed, which the hospice and palliative nurse can use to assist the patient and family to make informed decisions about care. Disease trajectories and the prognostic indicators that signal disease progression are reviewed, both in general and disease-specific. The impact of co-morbid conditions on prognosis is also presented. Finally, prognostication tools and their appropriate use to improve prognostication skills and to serve in documentation to support prognosis is explored.
